Tool for Incorporating Human Factors During Process Hazard Analysis (PHA) Reviews of Plant Designs provides additional human factors guidance and tools in the petroleum and petrochemical industry. Use of the Tool is intended to help eliminate incidents by identifying and mitigating human factors issues during process hazard analysis. It will also assist facilities to meet the requirements of OSHAs Process Safety Management (PSM) and EPAs Risk Management Plan (RMP) rules for addressing human factors during hazard reviews. The Tool uses screening questions, example situations, and potential solutions, and focuses on operations that can be hazardous due to human factors implications. See more
